Optimization of vonoprazan‐amoxicillin dual therapy for eradicating Helicobacter pyloriinfection in China: A prospective, randomized clinical pilot study

Abstract

Background

Vonoprazan-amoxicillin (VA) dual therapy has been shown to achieve acceptable cure rates for treatment of Helicobacter pylori(H. pylori) in Japan. Its effectiveness in other regions is unknown. We aimed to explore the efficacy of VA dual therapy as first-line treatment for H. pyloriinfection in China.

Methods

This was a single center, prospective, randomized clinical pilot study conducted in China. Treatment naive H. pyloriinfected patients were randomized to receive either low- or high-dose amoxicillin-vonoprazan consisting of amoxicillin 1 g either b.i.d. or t.i.d plus VPZ 20 mg b.i.d for 7 or 10 days. 13C-urea breath tests were used to access the cure rate at least 4 weeks after treatment.

Results

Three hundred and twenty-three patients were assessed, and 119 subjects were randomized. The eradication rates of b.i.d. amoxicillin for 7 and 10 days, t.i.d. amoxicillin for 7 and 10 days were 66.7% (16/24), 89.2% (33/37), 81.0% (17/21), and 81.1% (30/37) (p = .191) by intention-to-treat analysis, respectively, and 72.7% (16/22), 89.2% (33/37), 81.0% (17/21), and 81.1% (30/37) (p = .454) by per-protocol analysis, respectively.

Conclusion

Neither 7- or 10-day VA dual therapy with b.i.d. or t.i.d. amoxicillin provides satisfied efficacy as the first-line treatment for H. pyloriinfection in China. Further optimization is needed.

Development of a targeted gene panel for the diagnosis of Gorlin syndrome

A rare, inherited disorder that affects many organs and tissues in the body. People with this disorder have a very high risk of developing basal cell skin cancer during adolescence or early adulthood. They are also at risk of developing medulloblastoma (a type of brain cancer) and other types of cancer.

In this study, a gene panel was developed to overcome the challenges in the diagnosis of Gorlin syndrome and allow diagnosis using a single test. A custom panel was generated for four genes associated with Gorlin syndrome: PTCH1, PTCH2, SMO, and SUFU. Selumetinib in Children with Neurofibromatosis Type 1 and Asymptomatic Inoperable Plexiform Neurofibroma At Risk for Developing Tumor-Related Morbidity

Abstract
Background
Selumetinib was recently approved for treatment of inoperable symptomatic plexiform neurofibromas (PNs) in children with neurofibromatosis type 1 (NF1). This parallel phase II study determined the response rate to selumetinib in children with NF1 PN without clinically significant morbidity.
Methods
Children with NF1 and inoperable PNs, which were not yet causing clinically significant morbidity but had the potential to cause symptoms, received selumetinib at 25 mg/m 2 orally twice daily (1 cycle=28 days). Volumetric magnetic resonance imaging analysis and outcome assessments, including patient-reported (PRO), observer-reported, and functional outcome measures were performed every 4 cycles for 2 years, with changes assessed over time. A confirmed partial response (cPR) was defined as PN volume decrease of ≥20% on at least 2 consecutive scans ≥3 months apart.
Results
72% of subjects experie nced a cPR on selumetinib. Participants received selumetinib for a median of 41 cycles (min 2, max 67) at data cut-off. Approximately half of children rated having some target tumor pain at baseline, which significantly decreased by pre-cycle 13. Most objectively measured baseline functions including visual, motor, bowel/bladder or airway function were within normal limits and did not clinically or statistically worsen during treatment.
Conclusions
Selumetinib resulted in PN shrinkage in most subjects with NF1 PN without clinically significant morbidity. No new PN related symptoms developed while on selumetinib, and PRO measures indicated declines in tumor-related pain intensity. This supports that selumetinib treatment may prevent the development of PN related morbidities, though future prospective studies are needed to confirm these results. (ClinicalTrials.gov NCT01362803)

Infant Growth Trajectories and Lipid Levels in Adolescence: Evidence from a Chilean Infancy Cohort

Abstract
Growth in early infancy is hypothesized to affect chronic disease risk factors later in life. To date, most reports draw on European ancestry cohorts with few repeated observations in early infancy. We investigated the association between infant growth before six months and lipid levels in adolescents in a Hispanic/Latino cohort. We characterized infant growth from birth to five months in males (n=311) and females (n=285) from the Santiago Longitudinal Study (1991-1996) using three metrics: weight (kg), length (cm) and weight-for-length (WFL) (g/cm). SuperImposition by Translation and Rotation (SITAR) and latent growth mixture models (LGMM) were used to estimate the association between infant growth characteristics and lipid levels at 17 years. We found a positive relationship between the SITAR length velocity parameter before six months of age and high-density lipoprotein cholesterol levels in adolescence (11.5, standard error=4.1) indicating hi gher high-density lipoprotein cholesterol levels occurring with faster length growth. The strongest associations from the LGMM models were between higher low-density lipoprotein cholesterol and slower WFL growth, following a pattern of associations between slower growth and adverse lipid profiles. Further research in this window of time can confirm the association between early infant growth as an exposure and adolescent cardiovascular disease risk factors.
