There is no doubt that the Presidential election has been on the forefront of political news this year. The latest on the subject mentioned by multiple media outlets surround the possibility of Former Arizona Governor Jan Brewer joining Donald Trump's national ticket as the vice-presidential pick. So where is the connection to members of the American Sleep and Breathing Academy (ASBA)? David Gergen, Executive Director of the ASBA, has remained close friends with Jan Brewer and the family for decades including the duration of her six year service to Arizona. This adds to the already vast reach the ASBA has in the political world and provides an additional valuable connection for ASBA members.
Gergen hosted the largest sleep apnea awareness event in January which attracted not only 3,000 attendees, but future Hall of Fame Larry Fitzgerald, Phoenix Police Department and Gergen's high school friend Arizona Attorney General Mark Brnovich. Subsequently, Mr. Brnovich was treated for obstructive sleep apnea (OSA) by ASBA Diplomate Dr. Castle at Koala Sleep Center of Arrowhead.
The ASBA has long held strong footing associated with congress and the US Department of Transportation. This has clearly been shown by current involvement in the process of establishing protocol for the transportation industry to be treated exclusively by ASBA members; David Gergen commissioned Dr. Elliott Alpher, ASBA Diplomate, earlier this year to launch an initiative in collaboration with Congresswoman Anna G. Eshoo to make oral appliance therapy the benchmark treatment for OSA.
As a result, a special committee of ASBA Diplomates and other powerful figures such as NFL greats Roy Green, Eric Dickerson, Marcus Allen, Congressman Marty Russo, Congressman Jim McGovern, Congresswoman Anna Eshoo, and Senator Chris Dodd will be sent back to Washington to speak to the Senate on September 27, 2016.
Dr. Elliott Alpher says, "The ASBA is the only academy fighting for the dentists' rights in the sleep apnea arena and the efforts will become stronger as the American Sleep and Breathing Academy grows. Mistuning detection performance of ferrets in a go/no-go task

The harmonic structure of sounds is an important grouping cue in auditory scene analysis. The ability of ferrets to detect mistuned harmonics was measured using a go/no-go task paradigm. Psychometric functions plotting sensitivity as a function of degree of mistuning were used to evaluate behavioral performance using signal detection theory. The mean (± standard error of the mean) threshold for mistuning detection was 0.8 ± 0.1 Hz, with sensitivity indices and reaction times depending on the degree of mistuning. These data provide a basis for investigation of the neural basis for the perception of complex sounds in ferrets, an increasingly used animal model in auditory research.

Integration of partial information for spoken and written sentence recognition by older listeners

Older adults have difficulty understanding speech in challenging listening environments. Combining multisensory signals may facilitate speech recognition. This study measured recognition of interrupted spoken and written sentences by older adults for different preserved stimulus proportions. Unimodal performance was first examined when only interrupted text or speech stimuli were presented. Multimodal performance with concurrently presented text and speech stimuli was tested with delayed and simultaneous participant responses. Older listeners performed better in unimodal speech-only compared to text-only conditions across all proportions preserved. Performance was also better in delayed multimodal conditions. Comparison to a younger sample suggests age-related amodal processing declines.

Is markerless acquisition technique adequate for speech production?

In this study, the precision of markerless acquisition techniques have been assessed when used to acquire articulatory data for speech production studies. Two different markerless systems have been evaluated and compared to a marker-based one. The main finding is that both markerless systems provide a reasonable result during normal speech and the quality is uneven during fast articulated speech. The quality of the data is dependent on the temporal resolution of the markerless system.

The effect of nocturnal wear of dentures on the sleep quality: a systematic review and meta-analysis

Abstract

Purpose

The effect of nocturnal wear of denture on sleep quality and integrity is still not well understood. Therefore, this systematic review was conducted to provide evidence on this topic.

Methods

Electronic searches were conducted from 1964 up to September 2015, using MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials. Only publications in English or French, in which sleep quality of edentate adult individuals wearing dentures at night was compared to those not wearing were included in the review. Random effects models were used to pool the effect sizes.

Results

A total of 10 studies were included in the systematic review and 5 in the meta-analysis. No statistically significant difference between sleeping with denture and without denture was found for the Apnea-Hypopnea Index (AHI; Standard Mean Difference = −0.60, 95 % CI −1.67–0.47; Z = −1.10; p = 0.27). However, there was considerable heterogeneity in the studies included in the meta-analysis (Tau2 = 1.34; Q-value = 59.32, df = 4 (P < 0.0001); I2 = 93.3 %). When results from randomized controlled trials (RCTs) were pooled in subgroup analyses of AHI, there was a tendency towards favoring sleeping without dentures (P = 0.059) and no evidence of heterogeneity between studies (Tau2 = 0.000; Q-value = 0.06, df = 1 (P = 0.80); I2 = 0.000 %).

Conclusion

The current evidence suggests that there is no difference in the sleep quality and integrity of individuals wearing or not wearing their denture during sleep. However, the results of randomized controlled trials favoring sleeping without dentures and the likely presence of bias in the previous studies indicate the need for further randomized controlled trials for the development of clinical guideline.

Abstract

Purpose

The objective of this study was to evaluate endothelial function and carotid intima media thickness (CIMT) in moderate to severe obstructive sleep apnea (OSA) without comorbidities.

Methods

It is an observational case control study in which endothelial function was assessed using flow-mediated dilatation (FMD) and peripheral arterial tonometry (PAT), and carotid artery ultrasound was used to measure CIMT in study group subjects that included 20 normotensive, non-diabetic, treatment naïve, and moderate to severe OSA patients, and 20 normotensive, non-diabetic, and non-OSA subjects served as a control group. Study was conducted in Polysomnography Laboratory, Department of Internal Medicine, All India Institute of Medical Sciences (AIIMS) Hospital, New Delhi.

Results

FMD was significantly lower in the moderate to severe OSA group compared to non-OSA group (mean ± SD, 8.3 ± 2.8 vs. 13.4 ± 4.1 %; p = 0.0001). Reactive hyperemia index (RHI) was also significantly lower in the OSA group (1.55 ± 0.27 vs. 2.01 ± 0.48, p = 0.0007). CIMT was observed to be significantly higher in the OSA group compared to the non-OSA group (0.54 ± 0.09 vs. 0.48 ± 0.08 mm; p = 0.049). In the OSA group, FMD, RHI, and CIMT did not show a significant correlation with OSA disease severity indices [apnea hypopnea index (AHI), oxygen desaturation index (ODI), and minimum O2 saturation].

Conclusion

Endothelial function in macrovascular and microvascular circulation is significantly impaired in moderate to severe OSA patients without comorbidities. These patients also show evidence of subclinical atherosclerosis, in the form of increased CIMT.

Abstract

The surgeons performing rhinoplasty found Graft selection the greatest challenge. To avoid an immune response the preferred choice thus far for nasal reconstruction would be autograft compared to allograft due to its lower rate of rejection. We have evaluated 30 patients who underwent open rhinoplasty and We used conchal and septal cartilaginous grafts in various forms by the open approach to correct various nasal deformities compared our experience regarding the operative technique, graft availability, indications, and limitations. No bony graft material or synthetic materials were used. Preoperative extensive evaluation of the patient was done in order to determine the type of deformity and the type of graft to be used in order to correct the deformity. Preoperative and postoperative photographs were taken in four basic views: frontal, lateral, lateral-oblique and basal in order to assess the results of the surgery. The study was done on 30 patients (20 male and 10 females) using the external rhinoplasty approach using the septal and conchal cartilages in different forms. Autologous septal cartilage was used in most of the patients (25 out of 30) and conchal cartilage was used in 5 patients. Multiple grafting techniques were used in some patients. Three patients had traumatic etiology. Columellar strut graft along with TIG technique was used in 16 patients, spreader graft was used in 8 patients, and septal extension graft was used in 5 patient and shield graft in 1 patient. Septorhinoplasty continues to evolve through various new techniques and modifications with the main goal to improve functional nasal airway and to restore cosmetic harmony to the face. Optimum result is very much dependent on the surgeon's attention to functional, aesthetic, and reconstructive principles and graft selection.

This study deals with the classical problem of sound radiation of an excited clamped circular plate embedded into a flat rigid baffle. The system of the two coupled differential equations is solved, one for the excited and damped vibrations of the plate and the other one—the Helmholtz equation. An approach using the expansion into radial polynomials leads to results for the modal impedance coefficients useful for a comprehensive numerical analysis of sound radiation. The results obtained are accurate and efficient in a wide low frequency range and can easily be adopted for a simply supported circular plate. The fluid loading is included providing accurate results in resonance.

We are reporting on a patient who underwent stapedotomy bilaterally 20 years ago and had sensorineural and mixed hearing loss. Recurrent otitis externa prevented the use of hearing aids. Abstract

Mounting an immune response sufficient to eradicate a tumor is the goal of modern immunotherapy. Single agent therapies with checkpoint inhibitors or costimulatory molecule agonists are effective only for a small portion of all treated patients. Combined therapy, e.g., CTLA-4 and PD-1 checkpoint blockade, is a more effective treatment modality, but in preclinical studies OX40 agonism with CTLA-4 blockade using monoclonal antibodies (aOX40/aCTLA-4) failed to induce tumor regression of larger, more established tumors. We hypothesized that administration of a vaccine with a tumor-associated antigen targeted to the appropriate antigen presenting cell could make combined aOX40/aCTLA-4 therapy more effective. We administered an antibody-based vaccine targeting HER2 to the DEC-205 endocytic receptor on cross-presenting dendritic cells (anti-DEC-205/HER2; aDEC-205/HER2) and a potent adjuvant (poly (I:C)) to assist with maturation, along with aOX40/aCTLA-4 therapy. This therapy induced complete regression of established tumors and a pronounced infiltration of effector CD8 and CD4 T cells, with no effect on regulatory T cell infiltration compared to aOX40/aCTLA-4 alone. To be maximally effective, this therapy required expression of both OX40 and CTLA-4 on CD8 T cells. These data indicate that vaccination targeting cross-presenting dendritic cells with a tumor-associated antigen is a highly effective immunization strategy that can overcome some of the limitations of current systemic immunotherapeutic approaches that lack defined tumor-directed antigenic targets.

Abstract



Background

We have previously investigated neoadjuvant ipilimumab (ipi) for patients with locally/regionally advanced melanoma. That initial assessment of peripheral blood mononuclear cells (PBMC) showed a significant increase in shared tumor associated antigen specific CD4+ and CD8+ T cell activation. We also observed a transient increase in circulating T regulatory cells (Treg) with a parallel increase in total CD4+ T cells, as well as a significant decrease in circulating myeloid derived suppressor cells (MDSC). The increase in circulating Treg frequency, as assessed at 6 weeks after initiation of ipilimumab, was significantly associated with improved progression free survival (PFS, p = 0.034; HR = 0.57) and returned to baseline levels by 12 weeks. To shed light on the unexpected positive correlation between increased Treg and PFS, we here investigated the suppressive activity of circulating Treg at baseline and 6 weeks.




Methods

Patients were treated with ipi (10 mg/kg intravenously every 3 weeks for 2 doses) bracketing definitive surgery. Treg (CD4+CD25+CD127dim/-) were isolated from pre-ipi (baseline) and post-ipi (6 weeks) PBMC samples. Treg were co-cultured with autologous responder CD4+ T cells that were stimulated with OKT3/IL-2/CD28 and CFSE-labeled T cells. 1:1, 1:2, and 1:5 ratios were tested. Flow cytometery was used to evaluate the degree of Treg proliferation suppression.




Results

Thirty-five patients were enrolled in the study; 18 patients had adequate PBMC samples with sufficient Treg isolated for Treg functional analysis. At 6 weeks following ipi, a decrease in percent of maximal inhibition of Th by Treg compared to baseline was seen for some patients. Scatter plot analysis showed no association between Treg frequency and function at any ratio or between circulating Treg frequency and function at baseline and at 6 weeks post-ipi. An increase in Treg suppressive function was significantly associated with a decrease in PFS (p = 0.02).




Conclusions

We find that Treg frequency measures do not correlate with suppressive activity measured ex vivo. Treg suppressive activity increases correlate with poorer patient outcomes.
